Positions: CNA, LPN, RN
(CNA): $24-$25/hour
(LPN): $34-$36/hour w/ benefits or $36-$38/hour w/out benefits
(RN): $45/hour w/ benefits or $48-$50/hour w/out benefits
All shifts available for CNA, LPN and RN roles: 7am-3pm, 3pm-11pm, 11pm-7am
Required Documents (Please Bring)
- Valid New York State CNA certification or LPN/RN License and Registration
- Valid photo ID
- Social Security Card
- Resume
- Two reference letters
- Physical exam (within last year): two step PPD OR Quantiferon lab reports (within last year)
- proof of MMR vaccination (two doses) OR MMR Titers lab reports
- COVID-19 vaccine record (2 doses Pfizer/Moderna or 1 dose Janssen)
- voided check or direct deposit enrollment form
- LPN/RN: educational degree & BLS
